While trying to find out more about what led Watson to leave her old job, Holmes begins to suspect that there’s something fishy about patients at the hospital supposedly dying of natural causes, leading to an elusive Angel of Death. Meanwhile, Joan meets up with her old doctor friend, gives in to her first instinct as a doctor and saves a girl’s life.
